Wraith Madoka is a mutant wraith from the Wraith Arc of Puella Magi Madoka Magica manga. Despite being a wraith, she is the only one that is not a direct antagonist and rather helps Homura Akemi and the other magical girls.

After Homura gets all of her magic absorbed from the wraiths, a bunch of mutant wraiths resembling characters that have died start appearing around the magical girls, with the one resembling Madoka Kaname appering before Homura Akemi, though this wraith was noticeably different from the others, not attacking Homura Akemi and instead asking her help in defeating the giant wraith that stole her magic.
She explains that Wraiths collect the emotional energy of humans and nullifies it to maintain a balance in the world as too much emotional energy released can cause the world and the heavens to flip, however through absorbing Homura's magic, the witch in her shield, Kriemhild Gretchen, had curse energy that they could not nullify and started corrupting the wraiths.
Homura agrees to help her, with them looking for the witch, after they fail to find it, Wraith Madoka reveals an unsettling truth to Homura. Her feelings and love for Madoka Kaname are gone. After revealing this fact, Homura starts to question if Madoka Kaname ever even actually existed, falling to despair and asking Wraith Madoka to take all of her emotions, with Wraith Madoka doing so. After this, the other magical girls, Mami Tomoe and Kyoko Sakura are busy fighting against Mutant Wraiths when Wraith Madoka comes to help them, showing them to be the only unique wraith willing to help the magical girls.
She explains to both of the magical girls what the giant wraith is and how much of a danger and threat it is to the world, with her going to fight it together with Mami after Kyoko dies from the Spindle Witch grabbing her and sucking out her soul. During the fight Wraith Madoka cannot defeat the Spindle Witch and ultimately gets destroyed, re-awakening Homura's feelings for Madoka and sending her into her soul gem where she directly speaks with her own magic.
There it's revealed by her own magic that she was mainly possessing the Madoka wraith which is why it acted as an ally and planned to have it be destroyed so the magic could be returned to Homura, for this she needed to take all of the magic out of Homura.

Trivia
Despite the actual Madoka Kaname prior to becoming one with the Law of Cycles not knowing about Homura Akemi's love for her, the Wraith Madoka knows this very well, very likely due to having Homura's magic inside of her.
Wraith Madoka was based on the image of Madoka Kaname that was inside of Homura Akemi's own mind, with her constant changing personality being due to Homura struggling to know what her idea of a normal Madoka would be due to her idolization of her.
